Annual surveys of space occupancy/use are conducted to maintain accurate records and to facilitate the reporting process. A space request form for space assignment/reassignment can be obtained at www.eiu.edu/~physplnt/spacform.shtml. This form should be used for the following purposes:

• Changes of room occupancy within space currently assigned to a department/unit
• Room use changes (i.e., office to classroom, classroom to laboratory, classroom to office)
• When a department/unit wants to transfer a room to another department/unit or wants to allow a faculty/staff member primarily assigned to another department to occupy a room presently under their control
• When a college/support unit wishes to transfer a room to another college/support unit or wants to allow a faculty/staff member primarily assigned to another area to occupy a room presently under their control
