The visual/audio release form is for use by any university department or Recognized Student Organization (RSO) that desires to obtain and use an individual's image or voice in any medium such as photograph, videotape, film, audiotape, or electronic, for educational purposes.
All executed consent forms should be retained indefinitely by the department, faculty member, RSO adviser, or other designee, until such time as it is required because of a claim or for three years after the photograph, videotape, film, audiotape, electronic use, or other medium, has been properly disposed of or destroyed.
