1. Requests must be submitted at least ten working days in advance
2. Services are available on a first come, first serve basis.
3. UB equipment can only be rented by EIU departments or Registered Student Organizations.
4. EIU departments and RSOs assume responsibility for any damaged, lost, or stolen equipment.
5. Payments must be made prior to the event. (Make checks payable to EIU).
6. UB productions reserves the right to refuse service for any reason (equipment availability).
7. Any request submitted less than three working days prior to the event will be automatically rejected.
8. Only equipment that is selected on this form or e-mailed before the event to the e-mail address above will be provided.
All payments or deposits must be made at least one week in advance. Make payments to Laurel Fuqua inside the Student Activities Center (2nd floor, University Union).
If at any point you need to make a change to an event that has already been submitted, please do not complete this form. Instead, please send an e-mail to firstname.lastname@example.org
