EIU Student WIM Information

Please complete the information at this site to attend the WIM conference for 2004 with EIU.

1. Print and fill out the Registration form. Send this form with $45.00 by February 1st.

2. Complete the Liability form and return it to Dr. McCallister. Make sure two witnesses sign beside your signature.

3. Contact Dr. McCallister to let her know that you are going. E-mail: cfpat@eiu.edu. Phone: 581-6351. When leaving a message, please speak slowly and clearly.

4. Make plans for your housing and for transportation to the conference. If you need other roommates at the Palmer House, please let me know. The deadline for the Palmer House is February 1, 2004. I will be taking a train up on Wednesday at 6 p.m. and returning on Saturday on the 4 p.m. train. I will be glad to pick you up and give you a ride to the Mattoon Train Station.
