Identifying Instructional Equipment Needs

In an effort to identify department instructional equipment needs, we are requesting that you complete the Media Services Instructional Equipment Request Form and return it to your Dean. Please use one form for each equipment item or system.
Chairpersons should rank each request from their department in priority order and forward all requests to their Dean. The Deans will then rank departmental requests and forward them to Media Services by date inserted here .
Media Services will combine all requests for a unified list of instructional equipment needs sorted in priority order. If you have any questions, please call 7564.
